• clicked a link on this site that was incorrectly formed
• are attempting to browse directories on the site
• are screwing around with my POST variables
• requested a file type that is no longer supported
• are trying to figure out how the formed links actually works
• are otherwise poorly attempting to hack my website
• you have submitted a blank form that requires data

This is not necesarily a 404 error because you may be requesting a file that actually does exist but has been made to appear as if it doesn't. This page is also occasionally used as a placeholder for an index page of an open directory, to further confuse people who are looking at things they shouldn't be.
